HHSC Licensed Medicaid Certified
For Adults & Seniors

Skilled Nursing Care

When someone needs medical care but wants to heal at home, our licensed nurses bring the clinical skill of a facility into familiar surroundings. Every skilled nursing visit is ordered by and coordinated with your physician, then carefully documented so your whole care team stays in sync — and you always know what's happening and why.

Who it's for: Adults and seniors recovering from surgery or a hospital stay, managing a chronic condition, or needing ongoing clinical monitoring at home.

Includes:
  • Wound care and dressing changes
  • Medication management and administration
  • Vital signs monitoring and health assessments
  • Post-surgical and post-hospitalization recovery support
  • Chronic disease management

For Children

Pediatric Home Health

Children with medical needs deserve care built around them — gentle, specialized, and closely coordinated with the people who know them best. Our pediatric team supports children at home and works hand in hand with families and pediatricians, so care never feels overwhelming and your child stays in the place they're most comfortable.

Who it's for: Children with complex medical needs, ongoing therapeutic needs, or those transitioning home after a hospital stay.

Includes:
  • Specialized pediatric nursing care
  • Coordination with pediatricians and specialists
  • Family education and support
  • Ongoing developmental and health monitoring
For Daily Living

Personal Assistance Services (PAS)

Not all support is medical. Personal Assistance Services help with the everyday tasks that keep someone safe, comfortable, and independent at home — delivered with patience and genuine respect for each person's dignity. For family caregivers, PAS also means dependable help and a chance to rest.

Who it's for: Individuals who need a hand with daily living, and the family caregivers who need reliable support alongside them.

Includes:
  • Bathing, dressing, and personal hygiene assistance
  • Meal preparation
  • Light housekeeping
  • Mobility assistance
  • Companionship and daily living support
