Starlight Home Health & Pediatrics is committed to treating everyone we serve with dignity and fairness. This notice explains your right to care free from discrimination, the free help we provide, and how to raise a concern.
Our Commitment
Starlight Home Health & Pediatrics complies with applicable federal civil rights laws and does not discriminate, exclude people, or treat them differently on the basis of race, color, national origin (including limited English proficiency and primary language), sex, age, or disability.
This commitment applies to everyone we serve and to everyone who seeks our services — patients, clients, children and their parents or guardians, and family members — in every part of our home health, pediatric, and personal assistance services.
Free Help We Provide
We provide free aids and services to people who need help communicating with us. If any of the following would help you, tell us — there is no charge, and asking will never affect the quality of your care.
For people with disabilities
- Qualified sign language interpreters.
- Reasonable modifications to our policies and procedures when needed for equal access to care.
- Written information in accessible electronic formats, large print, audio, or other formats.
- Other auxiliary aids and services that support effective communication.
For people whose primary language is not English
- Qualified interpreters, provided in a timely manner.
- Written information translated into other languages.
- Spoken (oral) interpretation of written materials.
You are not required to bring your own interpreter. We do not ask family members, friends, or — except in narrow situations allowed by law, such as an emergency — minor children to interpret for you. For more detail, see our Language Assistance / Notice of Availability.
How to Request Help
To ask for an interpreter, a translated document, a disability accommodation, or an accessible format, contact us using the information at the bottom of this page. Please let us know what you need and, if possible, the language or format that works best for you. We will arrange it as promptly as we reasonably can.
Raising a Concern With Starlight
If you believe Starlight failed to provide these services or discriminated against you, you may raise your concern with us directly. Ask for the Administrator, who reviews concerns of this kind. You will not be penalized, and no one will retaliate against you, for raising a concern.
Using our internal process is not required before you contact an outside agency. You may file a complaint directly with the U.S. Department of Health and Human Services at any time.
